New Patient Registration

 To register you need to be resident in our practice area. Please complete an application form available from the Reception Desk. If you are only here temporarily please inform the receptionist.

Permanent patients may express a preference to receive services from a particular doctor or nurse, either generally or in relation to any particular condition.

Whilst we shall endeavour to comply with any such preference we reserve the right to refuse such services if the Doctor/Nurse does not routinely perform the service in question within the practice.

 

Proof of Identity for New Patients

 

Patients registering with the Practice will be asked for proof of identity. A list of acceptable documents is given below.

 

               birth certificate

         marriage certificate

         medical card

         driving licence

         passport

         local authority rent card

         paid utility bills

         bank/building society cards/statements

         National Insurance number card

         current employment payslip

         letter from Benefits Agency/benefit book/signing on card

         identity papers from the Home Office

 

 

Practice Area

 

The geographical area covered by the practice is as follows:

Carlisle side of M6 from Junction 44 to Junction 42. A line from Junction 42, through Brisco (included in area) to join B5229 (Dalston Road) at minor road junction just south of Cummersale. Follow minor road to junction with A595 (Wigton Road). Take route of proposed Nothern Relief Road past Newby West (excluded from area), through Cornhill on B5307 (Thurston Road), then a line east of Kirkandrews-on-Eden, across the river and south of Cargo (excluded from area) to join M6 at Junction 44.
